Minor/Certificate Declaration Form

Procedure

  1. Open the form as a PDF document and print out a copy of the form.
  2. Fully complete the form, sign and date.
  3. Contact your college's academic advising office (view a list of current advisors and contacts) prior to submitting the form.
  4. Submit the completed and signed form to your college's academic advising office (view a list of current advisors and contacts) or to the Registrar's Office (MH 161).
