Immunizations

If you are born prior to January 1, 1957, you are exempt from the immunization requirement.

To complete your file, you will need to review the Immunization Policy and fill out the Mandatory Immunization Form, making sure you sign it. Both forms are available for download on the UCF Health Center Immunization Department Web page. Please submit your completed form back to the Immunization Department at UCF Health Services using the contact information provided on the department's Web page
